Thank you for choosing our Wellness for Life Program. As you know, in addition to the benefits already offered by Mayo Clinic Health Solutions, your policy may also qualify for Wellness Rewards. Wellness Rewards offers the possibility of reducing the cost of your insurance if the Program's qualification criteria are met, as described in your policy.
To receive this reduction, we need a Wellness for Life Program Qualification Form to be submitted not less than every two years."
